1) File an online Lost Property Report / FIR with the local police station. 2) Execute a notarized indemnity affidavit on ₹50/₹100 stamp paper stating the loss. 3) Publish a lost notice in a daily newspaper (if mandated by university bylaws). 4) Submit the application form along with duplicate degree fees (₹1,000–₹3,000) to the Controller of Examinations.
