Go to swavlambancard.gov.in and click on 'Apply for Disability Certificate & UDID Card'. Fill in your demographic coordinates, education, and employment status.
Select your disability from the 21 categories under the RPwD Act 2016 and choose your resident District Hospital / Chief Medical Officer (CMO) office for the physical assessment.
Upload a clear passport photo showing the disability, signature/thumb impression, and Aadhaar card (under 100KB).
Visit the designated district hospital on the scheduled board assessment day. Specialists (orthopedic, ophthalmologist, audiologist, or psychiatrist) evaluate and assign the disability percentage.
The CMO / Medical Superintendent signs and uploads the medical evaluation report onto the national portal.
Download the digital e-Disability Certificate and e-UDID card from the portal. A national embossed tamper-proof plastic UDID card is dispatched to your residential address via India Post Speed Post.
Update your address, mobile number, or apply for lost UDID card replacement:
Log into the portal using your UDID Number and Date of Birth.
Choose the modification or card replacement option.
Upload your new address proof or police complaint (for lost card).
The District Social Welfare Officer approves the update and dispatches a fresh card.

Download both your e-Disability Certificate and e-UDID Card carrying the digital signature of the CMO.

Track your UDID card application and hospital assessment status online:

Input your application reference ID or 12-digit Aadhaar number.
Check whether your application is pending CMO medical assessment, certificate issued, or track the India Post speed post consignment number.
The Unique Disability ID (UDID) Card is a single, national smart identity card issued by the Department of Empowerment of Persons with Disabilities (DEPwD) to Divyangjan. It eliminates the need to carry multiple hospital papers and is universally valid across all Indian states and Union Territories for government welfare, scholarships, jobs, and travel concessions.
The 21 disabilities include: Blindness, Low Vision, Leprosy Cured persons, Hearing Impairment (Deaf and Hard of Hearing), Locomotor Disability, Dwarfism, Intellectual Disability, Mental Illness, Autism Spectrum Disorder, Cerebral Palsy, Muscular Dystrophy, Chronic Neurological conditions, Specific Learning Disabilities, Multiple Sclerosis, Speech and Language disability, Thalassemia, Hemophilia, Sickle Cell disease, Multiple Disabilities, Acid Attack Victims, and Parkinson's disease.
Under Section 2(r) of the RPwD Act 2016, a person with not less than 40% of a certified disability has a 'Benchmark Disability'. They are legally entitled to: 1) 4% reservation in government jobs, 2) 5% reservation in higher education institutions, 3) Income tax deductions under Section 80U (₹75,000 for 40-79% disability, ₹1,25,000 for severe 80%+ disability), and 4) Scribe and 20 min/hour compensatory extra time in competitive exams.
As per Ministry of Social Justice guidelines, candidates with benchmark physical limitations in writing (locomotor, cerebral palsy, visual impairment) are entitled to: 1) Bring their own scribe or request the exam conducting body (NTA, UPSC, SSC, IBPS) to provide one, and 2) Receive compensatory extra time of minimum 20 minutes per hour of examination.
