Visit government hospital for disability assessment by medical board.
Medical board will assess disability percentage and issue certificate.

To update your Disability Certificate details — such as registered mobile number, address, or linked records — log in to the official portal and use the profile update or settings section.
Visit https://www.swavlambancard.gov.in and log in using your registered credentials or OTP.
Go to your profile or account settings section.
Choose the detail you want to change on your Disability Certificate.
Verify the change with an OTP sent to your registered mobile number or email.
Confirm and save the updated information. Changes typically take effect immediately.
You can download your Disability Certificate as a PDF or access it digitally through the official portal or ABHA / DigiLocker platform.
Visit https://www.swavlambancard.gov.in and sign in.
Navigate to your profile or the card/document section.
Select "Download Card" or "Export PDF" to get your digital copy.
Save on your device or upload to DigiLocker for secure cloud storage.
You can check the registration or enrollment status for your Disability Certificate by visiting the official portal and entering your registration number or Aadhaar.
Go to https://www.swavlambancard.gov.in and find the "Check Status" or "Verify" option.
Provide your registration ID, Aadhaar, or mobile number to look up your record.
Complete OTP verification and check your current enrollment or card status.
Unique Disability ID is a national database ID for persons with disabilities to access benefits.
Reservation in education and jobs, tax benefits, travel concessions, and various government schemes.
Permanent disabilities have lifetime validity. Temporary disabilities need renewal as specified.
Visit the Swavlamban Card portal (swavlambancard.gov.in) and click 'Apply for Disability Certificate & UDID Card'. Fill out the form, upload identity proof, a recent photo, and submit it. You will be scheduled for a medical assessment at a district hospital.
Candidates with a benchmark disability (usually 40% or more) involving locomotor disability or cerebral palsy, or those visually impaired, are legally allowed to use a scribe. A specific scribe declaration form and the UDID/Disability certificate must be submitted to the exam conducting body beforehand.
